Only saturation divers actually need one — but everyone wants the badge.

Saturation divers spend days in pressurised chambers breathing helium-oxygen mixtures. Helium molecules are tiny — they seep past gaskets and pressurise the inside of the case. On rapid decompression, the crystal can blow off.
A spring-loaded valve at 9 o'clock (Rolex Sea-Dweller, Omega Seamaster 600, Tudor Pelagos 39) opens automatically when internal pressure exceeds external pressure, venting the helium.
No recreational diver ever needs it. Saturation diving is done by ~5,000 commercial divers worldwide. For everyone else, an HEV is a decoration with a story.
